Mrs. Lockhart       2004-05              Algebra

Welcome to room 123!   Please read this carefully, as these are the policies that I will hold you responsible for.

Attendance: Attendance in class needs to be a priority.

These are the policies I will be following regarding attendance:

            1.  You must have absences excused in office.  Absences not cleared in office within 48 hours of absence are unexcused.

            2. Unexcused absences will result in zero credit for that day.

            3. Makeup work is due 2 days after an absence. Late work is not accepted for credit. It is your responsibility to get  assignments missed and turn them in on time. Assignments   will be posted daily on Thurston’s website: ths.sps.lane.edu

            4. A test missed must be a taken within a week of the absence  or you may receive a 0 on it.

Behavior:

          1. Be on time: Students arriving up to ten minutes late will

be considered tardy, after that, absent.   Every three tardies will result  in detention.

             2.  No food (candy included) or drink is allowed in class.

(during hot weather, water bottles are ok)

             3.  No electronic devices (headphones, cell phones, pagers)         These devices are not allowed in class. See p. 19 of student planner for school policy regarding this issue.

4. Bring your supplies each day: pencil ,paper, ruler,    scientific calculator, book.

             5. Be cooperative and respectful: This class needs to be a

place of quiet work. Rowdy behavior and excessive talking  will not be allowed. Students are expected to work on math  during the time they are in class.

The following discipline plan  will be followed for violations:

                                    A.  One verbal warning

                                    B.  Move seat - isolate and assign detention

                                    C.  Remove from classroom and call parent

                                    D.  Conference with counselor/parent

E.  Severe or chronic violations: referral to       office

Hall Passes: You will be allowed 5 hall passes this quarter. Once you have used your five, you won’t be allowed to leave class. Also, any that aren’t used will be counted as extra credit on your quarter grade, so use them wisely.
